What Is Abdominoplasty (Tummy Tuck)? Who Is It Suitable For?

Abdominoplasty, or tummy tuck surgery, is a surgical procedure performed to tighten the loosened muscles of the anterior abdominal wall and remove excess sagging skin in the area. The goal of this operation is to achieve a flatter, tighter, and more proportionate abdominal silhouette. This procedure addresses not only aesthetic concerns but also corrects the separation of the abdominal muscles known as diastasis recti and the functionally uncomfortable skin excess, significantly improving the quality of life for many patients.

Who Is Abdominoplasty Performed On?

Abdominoplasty is not a surgery that overweight individuals can turn to as a weight-loss method. It is suitable for individuals who, despite being at a normal weight, experience sagging and deformity in the abdominal area that they have been unable to correct through diet and exercise.

Rapid weight gain and loss, pregnancy, the aging process, genetic predisposition, and previous surgeries in the abdominal area are among the main causes that pave the way for abdominal muscle laxity and skin sagging. Individuals who have experienced one or more of these factors, are in good general health, do not smoke, and have kept their weight stable are considered suitable candidates for abdominoplasty.

It is recommended that patients who are planning a pregnancy postpone the surgery. Since the abdominal muscles and skin will stretch and loosen again during pregnancy, the results achieved after the operation may not remain permanent. The appropriate timing for surgery can be considered once the postpartum breastfeeding period has ended and body weight has stabilized.

Types of Abdominoplasty

The surgical technique is determined according to the degree and extent of deformation in the patient’s abdominal area. Three main approaches stand out:

  • Mini tummy tuck: Preferred in cases where sagging and fat accumulation are concentrated only below the navel. A relatively short incision made along the bikini line tightens the area below the navel. Compared to a full tummy tuck, it takes less time, leaves a more limited scar, and has a faster recovery process.

  • Full tummy tuck: Applied in patients with widespread sagging and deformation in the anterior abdominal wall. With an incision extending sideways from the bikini area, the entire abdominal skin is lifted up to below the rib cage, and the loosened muscles are tightened with sutures like an internal corset. After the excess skin and fat tissue are removed, the navel is repositioned. For widespread sagging over large areas, the full incision can be extended; when there is significant fat accumulation at the midline, the inverted-T-shaped Fleur-de-lis technique can be applied to achieve a pronounced waist contour.

  • Circumferential tummy tuck (360-degree tuck): Preferred when sagging in the waist and back area, in addition to the abdomen, is also to be treated. This method is often considered for patients who have developed significant excess skin over the entire body following sleeve gastrectomy or major weight loss. The incision starts at the bikini line and extends to encircle the entire body.

Pre-Operative Preparation

The pre-operative preparation process is of great importance in order to achieve a good result with abdominoplasty and to minimize the risk of complications.

  • Smoking and alcohol should be stopped at least two weeks before surgery; nicotine negatively affects wound healing and increases the risk of clotting.

  • Blood-thinning medications such as aspirin and ibuprofen should also be discontinued during the same period.

  • For those using oral contraceptives, it is recommended to pause this treatment 30 days before surgery.

  • Multivitamin supplements should not be used one week before surgery, and those containing vitamin K should not be used for 30 days after surgery.

  • Being at the hospital one day before surgery is important for the complete performance of the necessary pre-operative tests.

During the examination, the patient’s abdominal area is evaluated, measurements are taken, and the most suitable technique is determined based on individual anatomy.

The Surgical Process

Abdominoplasty is performed under general anesthesia. Depending on the technique used and any additional procedures, the operation takes two to four hours. In some cases, liposuction may be performed simultaneously with abdominoplasty to enhance the contouring effect.

Post-Operative Recovery Process

After the surgery, the abdominal area is covered with surgical dressings. Thin drainage tubes may be placed around the incision to prevent the risk of fluid accumulation. The main points to pay attention to during the recovery process are as follows.

  • The abdominal corset should be worn regularly for at least six weeks; this support helps reduce swelling and helps the tissues take on their new contours.

  • A balanced and adequate diet speeds up recovery.

  • Medications prescribed by the doctor should be used fully and in accordance with the instructions.

  • It is recommended to avoid excessive physical activity, movements that put pressure on the abdominal area, and hot baths.

  • Comfortable and loose clothing should be preferred during this period.

Risks of Abdominoplasty

As with any major surgical procedure, abdominoplasty also carries certain risks. These may include subcutaneous fluid accumulation known as seroma, infection, edema, difficulty in wound healing, scarring, temporary changes in skin sensation, and complications related to anesthesia. These risks vary depending on the technique chosen, the patient’s general health condition, and the quality of post-operative care.
